Job Description

Responsibilities:

• Deliver exceptional nursing care to patients in their homes.
• Conduct patient assessments, monitor vital signs, and administer medications.
• Develop, implement, and evaluate individualized care plans in collaboration with healthcare teams.
• Educate patients and families about treatment plans, disease management, and health maintenance.
• Maintain accurate patient records and communicate effectively with the healthcare team.
• Ensure compliance with healthcare regulations and standards.
• Pass Meds
• Process PCP orders
• Meet with home care practitioners
• Conduct weekly med cart audits
• Conduct file audits to ensure compliance with state regulations

Qualifications:

• Current and valid RN or LPN license in the state of Florida.
• CPR/BLS certification.
• Strong assessment and communication skills.
• Ability to work independently
• Compassionate and patient-focused approach to care.
• Reliable and available to work on weekends.
