Job Description
We are looking for a compassionate and dedicated Patient Care Manager to join our team, focusing on our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role is centered around empowering patients to manage their chronic conditions and improve their overall well-being, ensuring they receive the highest quality care. As a key member of the care team, you will provide personalized support, develop care plans, and guide patients in managing their health effectively.
The ideal candidate will be a Certified Medical Assistant (MA) or Licensed Practical Nurse (LPN) with a solid foundation in chronic disease management and patient education. You should be passionate about working with individuals to improve their health outcomes and have a keen eye for detail.
Key Responsibilities:
Comprehensive Patient Assessments:
Conduct thorough assessments to understand each patient's medical, psychological, and social needs.
Identify challenges to disease management and collaborate with patients to develop strategies for overcoming them.
Individualized Care Plans:
Work with a multidisciplinary team to create personalized care plans for each patient, involving physicians, pharmacists, and other healthcare professionals.
Regularly review and adjust care plans to meet the evolving needs of patients.
Patient Education and Engagement:
Provide education on managing chronic diseases, including medication adherence, monitoring vital signs, nutrition, exercise, and making lifestyle adjustments.
Use motivational interviewing and behavior-change strategies to support patients in achieving their health goals.
Care Coordination:
Facilitate smooth communication between patients, caregivers, and the healthcare team to ensure seamless care coordination.
Track referrals, test results, and follow up on treatment to prevent gaps in care.
Data Monitoring and Reporting:
Monitor RPM data to identify trends or health risks and escalate concerns to the care team when necessary.
Use data insights to inform patient interventions and improve overall health outcomes.
Documentation and Compliance:
Accurately document all patient interactions, assessments, and care activities within the electronic health record (EHR) system.
Ensure adherence to CMS guidelines and requirements for CCM and RPM programs.
Program Management:
Assist in enrolling eligible patients in CCM and RPM programs.
Monitor the effectiveness of these programs and gather feedback to identify areas for improvement.
Qualifications and Requirements:
Education & Certification:
Must be a Certified Medical Assistant (MA) or Licensed Practical Nurse (LPN).
Additional certifications in disease management, case management, or patient education are a plus.
Experience:
At least 2 years of experience in patient care, care coordination, or chronic disease management.
Prior experience with CCM or RPM programs is preferred but not required.
Technical Skills:
Proficiency in using electronic health records (EHR) systems.
Familiarity with Google Workspace and experience with RPM devices and data monitoring tools is beneficial.
Soft Skills:
Strong communication skills to effectively educate patients and collaborate with healthcare teams.
Excellent organizational and multitasking abilities, with a high attention to detail.
Work Environment:
A reliable internet connection and a HIPAA-compliant workspace are required for telephonic and virtual patient interactions.
