Your Mission
As a Telehealth Nurse Practitioner, you'll play a crucial role in evaluating and managing patients with conditions related to cardiometabolic health. Your responsibilities will include conducting video visits, preparing and documenting patient interactions, and collaborating with a multidisciplinary team to provide personalized and evidence-based care.
What You'll Do
Patient Care: Conduct video visits to evaluate and manage conditions such as hypertension, hyperlipidemia, diabetes, prediabetes, overweight, and obesity using evidence-based medicine.
Documentation: Prepare for and document visits clearly and concisely within 24 hours to ensure continuity of care with the multidisciplinary team.
Collaboration: Work closely with registered dietitians, health coaches, and other healthcare professionals to reinforce treatment plans and coordinate care.
Technology Use: Adapt to and utilize multiple technological platforms for patient care and communication.
Quality Metrics: Maintain high scores on process, quality, and patient satisfaction metrics.
Communication: Engage in asynchronous communication with patients and coordinate care with external providers as needed.
Meetings and Learning: Participate in weekly clinical meetings, educational sessions, and team meetings.
Who We're Looking For
Experience: At least 3 years of experience in primary care, cardiology, or endocrinology.
Licensing: Active nurse practitioner license in Texas, or in any of the following states: Arizona, California, Colorado, Florida, Illinois, Michigan, New Jersey, New York, Ohio, Texas, Virginia, Washington, Wisconsin.
Skills:
Passion for cardiometabolic health and evidence-based medicine.
Ability to deliver patient-centered care via video visits.
Strong communication skills for interacting with patients and team members.
Technical aptitude and adaptability to new technology platforms.
Team-oriented and flexible in a fast-paced environment.
