This position is a responsible professional position that reports to the Director of the Healthcare Quality and Clinical Management Unit for the Office of Children’s Medical Services Managed Care Plan. This position is responsible for promoting an integrated, comprehensive, and cost-effective health care system with a focus on health equity and social determinants of health to ensure access to quality care for CMS Plan enrollees. This incumbent will serve as the Enhanced Care Coordination (ECC) subject matter expert and will perform independent and complex work. This role is responsible for coordinating resources, leading implementing projects, and day-to-day coordination, oversight, and monitoring for efficient operation of Enhanced Care Coordination Program. This incumbent will be a Community Health Nursing Consultant for related CMS Plan Program initiatives and activities which include but are not limited to Care Coordination/Case Management and Enhanced Care Coordination.

 

Who We Are :

 

Florida Department of Health Children’s Medical Services (CMS) Managed Care Plan is responsible for the program oversight of the CMS Health Plan. CMS Health Plan provides benefits for children up to age 21 who have chronic and serious health condition(s).

 

Your Specific Responsibilities:

  • Performing monitoring activities that include case reviews, report reviews, leading initiatives for quality improvement projects/developing best practices and analyzing QI data in the area of Enhanced Care Coordination.
  • Analysis and trending of population data which includes Private duty nursing (PDN), Prescribed Pediatric Extended Care (PPEC), and skilled nursing facility (SNF) members.
  • Participating and ensuring CMS Plan vendor representation for all staffings, workgroups, and meetings related to ECC. Maintain tracking log of staffings and provide consultation on follow ups needed with the CMS Plan vendor.
  • Review and maintain quality of care complaints when escalated to the CMS Managed Care Plan Central Office for clinical reasons related to ECC.
  • Collaborating with the CMS Plan vendor to track and monitor disaster preparedness planning for CMS enrollees.
  • Tracking members transitioning and aging out of CMS within the year to ensure all protocols are followed and for making the CARES referral as applicable.
  • Performing program monitoring (desk and on-site reviews) to ensure that all requirements are being met in relation to the enhanced care coordination program.
  • Ensuring the CMS Plan vendor completes, and updates individualized transition plans as required by AHCA guidelines.
  • Assists the Healthcare Quality and Clinical Management Director with special projects as assigned.
  • Performs other related duties as assigned.

  • Proficient in Microsoft Office and strong computer skills;

  • Knowledge of enhanced care coordination principles, practices and techniques;
  • Ability to lead quality improvement projects and provide technical assistance

  • Ability to communicate effectively, both written and verbal;

Qualifications:

  • This position is required to have and maintain a clear and active Registered Nurse License in the State of Florida (F.S. Reference: 464) or Multistate Registered Nurse License from another Compact State.
  • Years of Experience: 1-3 Years of pediatric clinical experience preferred.
  • Travel time: 5%

Where You Will Work:

This is a telework position within the State of Florida. However, if located in these four counties (Leon, Jefferson, Gadsden, Wakulla) they are excluded from telework availability, but may be considered for a hybrid schedule.
