Job Description

We are seeking a compassionate and detail-oriented Case Manager Level II to support children and families through post-release services. In this role, you will utilize your experience in trauma-informed care, child welfare, and case management to create positive outcomes for the individuals you serve.

You'll have the opportunity to make a meaningful impact by conducting assessments, facilitating access to resources, and ensuring the safety and well-being of children and their sponsors. This position combines virtual and in-person work, requiring travel for home visits and assessments.

Key Responsibilities
Manage a caseload of post-release services (Level I & II) for children and sponsors.
Conduct comprehensive home studies to assess risk, safety, and placement suitability.
Collaborate with care providers to verify information and ensure appropriate sponsorship.
Evaluate sponsors' ability to provide a safe and supportive environment for children.
Provide ongoing case management services, including assessments, planning, and psychoeducation for children and their sponsors.
Utilize trauma-informed approaches and brief therapeutic techniques to address clients' needs.
Maintain detailed, confidential documentation while adhering to program regulations.
Report safety concerns, life changes, or significant incidents to program supervisors and child protection services as required.
Participate in training sessions and quality improvement initiatives.
Travel to conduct in-person home visits and assessments, occasionally working evenings or weekends to meet deadlines.

Qualifications
A Bachelor's degree in Social Work (BSW), Psychology, Sociology, Education, or a related field.
At least 1 year of experience managing cases in a related setting.
Strong report-writing skills and familiarity with child welfare policies.
Experience working with immigrant populations and providing trauma-informed interventions.
Proficiency in Microsoft Office and data entry systems.
A valid driver's license and availability for national travel.
